CONSENT to the processing of personal data
pursuant to Regulation (EU) 2016/679 of the European Parliament and of the Council (“consent”)
By completing any form on the www.cadmia3d.cz website and submitting it, I hereby grant Consent as
data subject (“Subject”) –
Consent is granted to the data controller (hereinafter referred to as CADMIA 3D), the business company CADMIA 3D s.r.o., with its registered office in Šternberk,
Nádražní 1958/48d, Postcode 785 01, ID No. 29451442
- The subject consents to the processing of his/her following personal data by CADMIA 3D and
other contractual processors (scope of processing):- identification and address data: title, academic title, first name, surname, date of birth, birth number,
address of permanent or temporary residence, delivery or other contact address, registered office, registration number - Electronic contact details: telephone, mobile phone, fax, e-mail address, mailbox ID
- other personal data related to the contractual relationship: bank account number
- other personal data provided by the subject in the Contract/Addendum or in other documents and negotiations, and
including subsequent updates

The subject consents to the processing of his/her personal data for the purposes set out below, to the extent specified below
(using the letters above) and for the period set out below (starting

subject’s request) and providing advice, in particular in the field of additive manufacturing and related

The subject gives consent for the entire scope of the data processed and for all the above purposes and their
respective scope. The subject may withdraw the consent given, in whole or in part, at any time, in the same simple
form as granted, in person at the headquarters of CADMIA 3D, by a trustworthy form of electronic communication or in writing
in paper form. The subject hereby consents to the above processing of his/her personal data, i.e.
by providing optional personal data for the above purposes. The subject also expressly declares that the consent of
to said processing is given free of charge and has not been conditional upon the provision of the service.
The consent granted is valid from the moment of its granting/signing – i.e. sending the completed form to
www.cadmia3d.cz, ordering, etc. CADMIA 3D will evaluate the scope of the data processed after the end of the contractual
relationship and limit it to what is necessary for the purposes of the processing.
The subject’s personal data is processed manually and automatically and may be disclosed
to CADMIA 3D employees, if necessary for the performance of their work duties, as well as to processors
with whom CADMIA 3D has concluded a contract for the processing of personal data and, where applicable, to other persons in accordance
with applicable law. The subject has the right to access, rectification, erasure (“the right
to be forgotten”), restriction of processing, portability, the right to object to processing and the right to lodge a complaint
with a supervisory authority, as well as the following information: the categories of personal data concerned; the purposes of the processing and their
legal basis incl., legitimate interests; the recipients or categories of recipients for disclosure (incl. Recipients in third
countries or international organisations); the intended period of storage of the personal data or, if this cannot be determined, the criteria
used to determine this period; information on the source of the personal data, if not obtained from the subject. An updated
list of personal data processors, as well as comprehensive information on the processing of personal data and a list of the rights and
obligations of the subject and CADMIA 3D, including an indication of the consequences of partial or total non-consent, are
published at the registered office of CADMIA 3D and will be provided to the subject upon request.
The subject declares to have read and understood this consent.
